Here are the ways you can deliver paperwork to TruStage Life of Canada. Be sure to consult your manager about funeral home location specifics regarding submitting paperwork.
Electronic Application System
After using the EAS, ensure that all enrollment form documents showing proof of signature are sent to TLOC.These documents should be sent to TLOC within 30 days of the application date.

- Original enrollment forms for all payment modes submitted with a completed cheque payable to "TruStage Life of Canada" must be mailed to the administrative office. Double-endorsed cheques will not be accepted.
- If a paper enrollment form has been properly completed, separate it as follows: White – TruStage Life of Canada Office | Yellow – Establishment | Gold – Applicant

- Faxed business must be printed in black ink only.
- Fund withdrawals must be drawn on your customer’s chequing/savings account or credit card — not from a line of credit.
- Withdrawal documentation can be either a voided cheque or a voided deposit slip (if the routing number is on the slip).
- A cheque payable to TLOC can only be accepted for a single premium draft where PAC has been selected on the form.
- After faxing new policies, keep original paperwork at the location rather than mailing to TLOC.
